Why the Hospital Charges Three Times More

The CT scanner inside a Fortis or Max hospital is the same Siemens, GE or Philips machine used at a standalone NABL-accredited diagnostic lab. The field strength is identical. The NABL accreditation requirements are identical. The radiologist has the same DNB or MD Radiology qualification. The digital report arrives in the same format within the same time window.

What the hospital charges for is everything else — the building, housekeeping, administrative departments, hospital-grade air conditioning, security, car parking, and a brand premium that is psychologically valuable but clinically irrelevant for a routine CT scan. None of these things improve the image quality of your scan or the accuracy of the radiologist's report.

There are situations where the hospital is the right choice: when your treating specialist specifically needs images loaded into their hospital's PACS system, or when you need immediate surgical backup alongside the CT — an emergency CT aorta, a CTPA for haemodynamically significant pulmonary embolism, or a post-trauma CT where intervention may follow immediately. For all routine outpatient CT scans a NABL-accredited standalone lab is clinically equivalent and 50 to 70% cheaper.

 

Plain CT vs CECT vs HRCT — What Your Prescription Actually Means

The single most important thing before booking is to follow your prescription exactly. Plain CT and CECT are not interchangeable and upgrading yourself is both unnecessary and expensive.

A plain CT scan uses no contrast injection and requires no preparation. You eat and drink normally. Plain CT is the correct investigation for CT brain in trauma, stroke and haemorrhage, for NCCT KUB in kidney stones (where contrast would actually obscure stones rather than improve their visibility), for HRCT chest in lung disease, for CT spine assessing bone, and for CT sinuses.

CECT — Contrast-Enhanced CT — uses an intravenous iodine contrast injection to highlight blood vessels, enhance organ tissue and identify pathological enhancement patterns in tumours and infections. It requires a four to six hour fast, a recent creatinine blood test to confirm adequate kidney function before contrast, and may require pausing metformin for 48 hours after the scan in diabetic patients. Contrast adds ₹1,500 to ₹3,000 to the cost and is the correct investigation for cancer staging, pulmonary embolism, acute abdominal infection and organ pathology requiring vascular mapping. HRCT chest — High Resolution CT — uses ultra-thin 1mm slices without contrast to image the lung parenchyma in detail. It is not a standard chest CT. It is specifically ordered for interstitial lung disease, pulmonary fibrosis, bronchiectasis, post-COVID ground glass opacities, sarcoidosis and early emphysema. No preparation is needed but patients need to hold their breath for several seconds during image acquisition. The CGHS rate of ₹2,300 makes this one of the most affordable specialist lung investigations available through the government health scheme.

 

CT Scan vs MRI — When CT Is the Right Choice

CT is preferred over MRI in specific clinical situations where its particular strengths — speed, bone detail, or lung parenchyma imaging — outweigh the absence of radiation that makes MRI attractive for other indications.

CT is clearly superior for kidney stones, where NCCT KUB is the gold standard and MRI is poor at stone detection. CT is superior for lung parenchyma assessment — HRCT chest provides detail of lung architecture that MRI cannot replicate. For acute trauma CT is preferred over MRI for its speed and ability to simultaneously assess brain, spine, chest and abdomen in a single rapid acquisition. CT Coronary Angiography — A Special Note on Price

CT coronary angiography deserves separate mention because it shows the widest price variation of any CT investigation and because several important technical factors affect whether the scan will be diagnostic.

At a standalone centre with a 256-slice CT scanner the price in Delhi is ₹8,000 to ₹12,000. The same scan at a private hospital with the same machine costs ₹15,000 to ₹30,000. The CGHS rate at NABH-accredited empanelled centres is ₹6,000 to ₹8,000.

Before booking CT coronary angiography confirm the slice count — 256-slice CT captures the entire heart in a single heartbeat and is significantly more reliable than 64-slice CT particularly in patients with higher resting heart rates or mild arrhythmia. Your cardiologist may prescribe a beta blocker to bring the resting heart rate below 65 bpm — take it as instructed. Fast for four hours and avoid caffeine for 12 hours before the scan. CT Scan Preparation by Scan Type

For plain CT — brain, spine, KUB, sinuses, HRCT chest — no preparation is needed. Eat and drink normally and remove metal objects before the scan.

For CECT with contrast — fast for four to six hours with water permitted freely. Bring a recent creatinine blood test ideally within three months. Diabetic patients on metformin should confirm with their doctor whether to pause it before and after contrast. Total appointment time is 45 to 60 minutes.
