The answer directly determines whether salvage radiotherapy, hormonal therapy, or systemic treatment is the right next step.

2. Biochemical recurrence after radiotherapy Rising PSA after radiotherapy — particularly in the context of PSA doubling time — warrants PSMA PET CT to distinguish local recurrence from regional or distant metastases. Local recurrence may be salvageable with focal treatment; distant metastases require systemic management.

3. Initial staging of high or intermediate-risk prostate cancer For newly diagnosed prostate cancer with Gleason score 7 or above or PSA above 10 ng/mL, PSMA PET CT provides superior lymph node staging compared with conventional CT — detecting metastatic lymph nodes too small to be identified on standard imaging. This changes the surgical plan in a meaningful proportion of patients.

4. Before Lu-177 PSMA therapy (PRRT for prostate cancer) Lu-177 PSMA is a targeted radioligand therapy for metastatic castration-resistant prostate cancer.
