Who Needs an Endoscopy? — Symptom Guide
Your gastroenterologist may recommend an endoscopy (gastroscopy or colonoscopy) if you are experiencing any of the following. Each symptom maps to the specific investigation most likely to be ordered.
Persistent acidity or GERD (acid reflux) If heartburn, acid reflux, or a burning sensation in the chest persists despite medication for more than 4–6 weeks, or if your doctor needs to rule out Barrett's oesophagus or H. pylori — gastroscopy is the investigation. H. pylori, if detected, is treatable and curable. Untreated it can lead to peptic ulcer disease and, in some cases, stomach cancer.
Stomach pain or discomfort Persistent upper abdominal pain, bloating, or a feeling of fullness after small meals — particularly in patients above 40 — warrants a gastroscopy to rule out gastric ulcer, gastritis, hiatus hernia, and early gastric cancer.
Difficulty swallowing (dysphagia) Difficulty or pain on swallowing food or liquids requires an urgent gastroscopy — it may indicate oesophageal stricture, achalasia, oesophageal cancer, or external compression. Do not delay investigation.
Vomiting blood or coffee-ground vomit This is an urgent presentation. Blood in vomit (haematemesis) or dark coffee-ground material indicates active or recent upper GI bleeding — an emergency gastroscopy is typically performed within 24 hours.
Black, tarry stools (melaena) Black stools with a distinctive foul smell indicate digested blood from an upper GI source — stomach ulcer, oesophageal varices, or gastric erosion. Requires urgent gastroscopy. Do not confuse with iron supplement-related dark stools — melaena has a distinctly tarry consistency and smell.
Blood in stools (rectal bleeding or haematochezia) Fresh red blood in stools or on toilet paper requires colonoscopy to rule out haemorrhoids, colonic polyps, inflammatory bowel disease (IBD), and colorectal cancer. Do not self-diagnose as haemorrhoids — colorectal cancer presenting as rectal bleeding is frequently missed this way.
Change in bowel habit Unexplained persistent change in bowel habit — constipation alternating with diarrhoea, or a change in stool consistency lasting more than 4–6 weeks — requires colonoscopy, particularly in patients above 40.
Unexplained weight loss Significant unintentional weight loss (more than 5% body weight over 6 months) with any GI symptoms warrants both gastroscopy and colonoscopy to rule out GI malignancy.
Anaemia without obvious cause Iron deficiency anaemia without an identified source in a patient above 40 requires both upper and lower GI endoscopy to rule out occult GI bleeding from polyps, cancer, or angiodysplasia.
Colon cancer screening Colonoscopy is recommended from age 45 for average-risk individuals in India, or from age 40 (or 10 years before the youngest affected family member) if you have a first-degree relative with colorectal cancer or adenomatous polyps.

Polyp Removal During Colonoscopy — Cost and What to Expect
Polyps are abnormal growths on the inner lining of the colon. Many are benign, but some are pre-cancerous adenomas that, if left in place, can develop into colorectal cancer over years. When polyps are found during colonoscopy, your gastroenterologist will typically remove them immediately in the same session — this is called polypectomy.

Capsule Endoscopy in Delhi — Complete Guide
Capsule endoscopy is the investigation that examines the small intestine — the 6–7 metre section of the digestive tract between the stomach and the colon that standard upper or lower endoscopy cannot reach.
How it works: You swallow a small disposable capsule — roughly the size of a large vitamin tablet — containing a wireless camera, light source, and battery. As it travels through the digestive tract over 8 hours, it captures approximately 50,000–60,000 images, which are wirelessly transmitted to a recording device you wear around your waist. A gastroenterologist then reviews the images to identify abnormalities.
Who needs capsule endoscopy:
- Unexplained gastrointestinal bleeding where gastroscopy and colonoscopy are both normal
- Suspected Crohn's disease involving the small intestine
- Small bowel tumours or polyps
- Coeliac disease evaluation and monitoring
- Surveillance in patients with polyposis syndromes (Peutz-Jeghers syndrome, familial adenomatous polyposis)
Why it costs more than standard endoscopy: The capsule itself is a single-use medical device imported from manufacturers such as Given Imaging (PillCam), Olympus, or Jinshan. The device cost alone accounts for a significant portion of the procedure fee — typically ₹10,000–₹15,000 of the total price. Reading and reporting 50,000+ images requires dedicated time from a specialist gastroenterologist using specific software. No sedation is needed but the recording device is worn for 8 hours.

Preparation: Fast for 12 hours before swallowing the capsule. A bowel preparation (laxative) the evening before is often recommended to clear the small bowel. Avoid strong magnets and MRI scans until the capsule has been excreted — usually within 24–48 hours.
Important limitation: Capsule endoscopy is diagnostic only — no biopsies can be taken and no therapeutic procedures are possible through the capsule. If a lesion is found, a follow-up endoscopy procedure may be required for tissue sampling or treatment.

Sedation for Endoscopy — Your Options
Gastroscopy without sedation Most patients tolerate gastroscopy without sedation. Mild throat discomfort and a gag reflex are expected but the procedure takes only 10–20 minutes. A local anaesthetic throat spray is applied. This is the most common approach at standalone centres and the most affordable.
Gastroscopy with conscious sedation IV midazolam ± fentanyl makes you drowsy and relaxed but not unconscious. Most patients have little or no memory of the procedure. Adds ₹1,500–₹3,000. Requires a companion to take you home.
Colonoscopy with conscious sedation Strongly recommended. Standard approach at most Delhi centres. Adds ₹1,500–₹3,000 to the base colonoscopy cost.
Propofol (deep sedation) You are fully asleep with no memory of the procedure. Requires an anaesthesiologist on site. Available at premium gastroenterology centres. Adds ₹2,500–₹5,000. Recommended for patients with high anxiety, previous difficult procedures, or complex long colonoscopies.
If you have any form of sedation, you must not drive or operate machinery for 24 hours. Arrange a companion before your appointment — centres will not administer sedation without a confirmed accompanying adult.
Complete Endoscopy Preparation Guide
Gastroscopy preparation
- Fast for 6–8 hours. No food, milk, or juice. Water usually allowed until 2 hours before — confirm with your centre
- Stop PPIs (omeprazole, pantoprazole), antacids, and H2 blockers 2 weeks before if investigating for H. pylori or Barrett's oesophagus — confirm with your gastroenterologist, never stop independently
- Continue blood pressure, heart, and diabetes medications with a small sip of water
- Remove dental plates or prostheses before the procedure
- Arrange a companion if having sedation
Colonoscopy preparation
- Bowel preparation begins the evening before — your gastroenterologist prescribes a PEG laxative (Peglec, Klean-Prep, or similar). Follow instructions exactly. Inadequate prep = rescheduled procedure
- Low-fibre diet for 2–3 days before — avoid seeds, nuts, raw vegetables, whole grains

- Blood thinners (aspirin, clopidogrel, warfarin, apixaban): your gastroenterologist advises whether to pause. Never stop independently
- Arrange a companion — sedation is strongly recommended
The most common reason for a rescheduled colonoscopy is inadequate bowel preparation. If your output is not clear, almost colourless fluid by the evening before, call your booking centre. Do not assume the preparation is sufficient if your output is still coloured or particulate.
What Happens During an Endoscopy
Gastroscopy: You lie on your left side. Throat spray applied. The endoscope — approximately 8–9mm in diameter — passes through your mouth, down the oesophagus, into the stomach and duodenum. You feel pressure or fullness, not pain. Biopsies cause no additional sensation. Takes 10–20 minutes.
Colonoscopy: You lie on your left side. The colonoscope is inserted through the rectum and advanced through the entire colon. CO₂ or air is introduced to open the bowel — causes bloating or cramping. With sedation, most patients experience minimal discomfort. Polyps found during the procedure can be removed immediately. Takes 30–60 minutes. Observation for 30–60 minutes after sedation before discharge.
After Your Endoscopy — Recovery and Post-Procedure Care
After gastroscopy
- Throat may be slightly sore for a few hours — normal, resolves quickly
- Eat and drink normally once throat numbness wears off — usually 30–60 minutes after the procedure
- Mild bloating or burping is normal
- If biopsy taken: avoid very hot food and alcohol for 24 hours
- Contact your gastroenterologist if: severe chest or abdominal pain, vomiting blood, black stools, or fever above 38°C within 48 hours
After colonoscopy
- Bloating and gas for several hours — walking helps
- Light easily digestible food for the first meal (soup, khichdi, curd). Resume normal diet the next day
- If polyps removed: avoid NSAIDs (ibuprofen, diclofenac, aspirin) for 1 week unless cleared by your gastroenterologist. Avoid strenuous activity for 24–48 hours
