If your ultrasound shows fatty liver — or your blood test shows high SGPT — this page tells you which test you need next, why a repeat ultrasound is usually the wrong answer, and what Fibroscan costs in Delhi in 2026.
Which Test Do You Need? — Quick Answer
|
Your Situation |
Test Needed |
Why |
|
Ultrasound shows Grade 1/2/3 fatty liver |
Fibroscan |
Ultrasound shows fat is present. Fibroscan measures how severe — fat quantity (CAP) and fibrosis (kPa). A repeat ultrasound adds no new information. |
|
Blood test shows high SGPT/ALT or SGOT |
Fibroscan + LFT panel |
Elevated enzymes indicate liver cell stress. Fibroscan stages the underlying damage. |
|
No test done yet, just concerned |
Ultrasound first |
Screens for fatty liver. If positive — Fibroscan next. |
|
Previous Fibroscan, following up |
Repeat Fibroscan |
Tracks disease progression accurately. Ultrasound cannot measure fibrosis progression. |
The most common mistake in Delhi: Patients told they have fatty liver on ultrasound return for a repeat ultrasound. Ultrasound can detect fat — it cannot detect inflammation or fibrosis. A repeat ultrasound after a fatty liver diagnosis tells you the same thing twice. Fibroscan is the investigation that changes clinical management.
Why Delhi Has a Specific Fatty Liver Problem
A study conducted specifically in urban Delhi found that 65.7% of urban adults aged 30–60 had fatty liver detected by ultrasound — and 16.5% had liver fibrosis detected by FibroScan transient elastography. Source: PLOS ONE, 2022. Watchdoq
Two in three urban Delhi adults have fatty liver. One in six has early liver scarring. Most have no symptoms.
Across India, the pooled prevalence of NAFLD is 38.6% among adults — approximately one in three Indian adults has the condition. Medifee
NAFLD has emerged as one of the leading causes of hepatocellular carcinoma and liver transplant in India.
Delhi NCR's risk profile compounds this further: sedentary IT workforce, high refined carbohydrate diet, and very high diabetes prevalence in North India. Among diabetic patients in northern India specifically, NAFLD prevalence reaches 72.4% — the highest of any Indian region studied. Source: Journal of Association of Physicians India, 2014. If you have diabetes, pre-diabetes, central obesity, or a sedentary lifestyle — and you have not had a fatty liver assessment — it is clinically overdue.
What Fatty Liver Grades Mean — And What They Cannot Tell You
Your ultrasound report grades fatty liver as Grade 1 (mild), Grade 2 (moderate), or Grade 3 (severe) based on how bright the liver appears on the scan image.
What the grade tells you: How much fat is visible on ultrasound.
What the grade cannot tell you: Whether you have liver inflammation (NASH), liver fibrosis (scarring), or early cirrhosis. These require Fibroscan.

Ultrasound Finding |
Meaning |
Next Step |
|
Grade 1 fatty liver / mild steatosis |
Mild fat in liver cells |
Fibroscan to rule out fibrosis. Lifestyle changes. |
|
Grade 2 fatty liver / moderate steatosis |
Moderate fat |
Fibroscan — fibrosis risk is meaningfully higher |
|
Grade 3 fatty liver / severe steatosis |
Significant fat |
Fibroscan urgently. Gastroenterologist review. |
|
Echogenic / hyperechoic liver |
Another term for fatty liver |
Same pathway — Fibroscan |
|
Hepatomegaly with fatty changes |
Enlarged + fatty liver |
Gastroenterologist + Fibroscan |
Important: "No focal lesion seen" in a fatty liver report means no tumour was detected — not that the liver is undamaged. It does not comment on fibrosis.

Ultrasound is the correct first investigation. Fibroscan is the correct second investigation once fatty liver is confirmed.
Fibroscan vs LFT (Liver Function Tests) — Which Measures What
Many patients confuse Fibroscan with LFT blood tests. They measure different things and are often done together, not instead of each other.
|
Test |
What It Measures |
What It Cannot Do |
|
LFT (SGPT, SGOT, bilirubin, albumin) |
Liver enzyme levels — indicates cell damage or stress |
Cannot measure fibrosis or fat accumulation |
|
Fibroscan |
Liver stiffness (fibrosis) + fat content (CAP) |
Cannot replace LFT for enzyme monitoring |
The correct combination: A Fibroscan + LFT panel together gives a complete liver assessment — structural damage (Fibroscan) and functional markers (LFT). Most EVE partner centres offer both in a single visit.

When to See a Gastroenterologist Directly
Book Fibroscan first in most cases. Go straight to a gastroenterologist — before testing — if any of these are present:
- Yellowish skin or eyes (jaundice)
- Swelling of abdomen or legs
- Easy bruising or unexplained bleeding
- Confusion alongside liver findings
- Ultrasound showing possible cirrhosis or suspicious nodule
These are signs of advanced disease requiring clinical assessment before investigation.
Clinical Note
From the reviewing gastroenterologist: The pattern I see most often in Delhi is a patient aged 35–55 who had a corporate health check, was told they have Grade 2 fatty liver, and came back for repeat ultrasounds every year for three years that showed the same thing. Ultrasound grading does not change meaningfully even when the underlying disease progresses — it is simply not sensitive enough to track fibrosis. The patients who have developed significant fibrosis undetected are almost always the ones who used ultrasound for follow-up instead of Fibroscan. A Fibroscan at ₹3,000–₹5,000 at a standalone lab in Delhi is a 10-minute, painless scan that gives two numbers — CAP and kPa — that actually change what your doctor recommends next. If your ultrasound has already shown fatty liver, the next investigation is Fibroscan, not another ultrasound. This is what Indian NASL guidelines recommend.
