Which PET CT Scan Do You Need?

Not all PET CT scans use the same tracer, and the tracer is chosen by your doctor to answer a specific question. Booking the wrong one means the scan cannot answer it.

FDG PET CT is the standard, used for around 85 percent of oncology scans. It detects cancer by its high glucose metabolism and covers detection, staging, restaging and treatment response across most cancer types. This is the ₹8,000 scan.

DOTANOC and DOTATATE PET CT target somatostatin receptors on neuroendocrine tumours, which often appear negative on FDG scans. This is a completely different mechanism for a different disease.

F-DOPA PET CT is used for specific neuroendocrine and neurological indications.

How to Prepare for an FDG PET CT

Preparation directly affects image quality, so it is worth getting right.

Fast for 4 to 6 hours before the scan, and avoid sugar, carbohydrates and caffeine for 24 hours beforehand. The FDG tracer competes with blood glucose, so high sugar levels genuinely degrade the scan. Water is not only allowed but encouraged. Avoid strenuous exercise for 24 to 48 hours, because active muscle takes up tracer and can obscure the images.

Diabetic patients need extra care. Bring a fasting blood sugar report, as glucose usually needs to be below 150 to 180 mg/dL for a diagnostic scan, and contact the centre in advance about insulin and medication timing. Do not assume standard preparation applies.

Bring your prescription and any prior imaging or kidney function report requested. Tell the centre if you are or may be pregnant, or breastfeeding.

What Happens on the Day

Expect to be at the centre for about 2.5 to 3 hours, most of which is not the scan.

After a small injection of the tracer into a vein, you rest quietly for roughly 60 minutes while it circulates and is taken up by tissues. You are asked not to move around or scroll on your phone during this uptake phase, because muscle and eye activity draw tracer away from where it is needed.

The scan itself takes 20 to 30 minutes. You lie on a table that moves slowly through the scanner, which is open at both ends, unlike an enclosed MRI. It is painless. Afterwards you can usually leave immediately, and drinking plenty of water helps flush the tracer out.

Is It Safe?

Yes. The radiation from a whole-body FDG PET CT is approximately 7 to 8 mSv, and the tracer has a very short half-life, clearing within hours, faster still with good hydration. The injection is the only uncomfortable part, and briefly. Pregnant and breastfeeding women should flag this in advance, as specific precautions apply.
