What is Fine Needle Aspiration Cytology (FNAC)?

FNAC is a type of biopsy where a very thin, hollow needle is inserted into the suspicious area. Unlike a core biopsy which removes a small piece of tissue, FNAC primarily collects individual cells or small clusters of cells. These cells are then spread onto glass slides, stained, and examined under a microscope. The procedure is often performed in an outpatient setting and typically requires no extensive recovery time.

Why is FNAC Recommended?

Your doctor may recommend an FNAC procedure if you have a palpable lump or mass that needs further investigation. Common areas where FNAC is performed include:

  • Breast Lumps: To evaluate suspicious breast masses.
  • Thyroid Nodules: To assess thyroid gland swellings.
  • Lymph Nodes: For enlarged lymph nodes in the neck, armpit, or groin.
  • Salivary Glands: To investigate swellings in glands like the parotid or submandibular.
  • Soft Tissue Masses: Lumps under the skin or in muscles.
  • Other Organs: In some cases, FNAC can be guided by ultrasound or CT scan for deeper lesions.

Preparation for Your FNAC Procedure

One of the advantages of FNAC is that it generally requires no special conditions or extensive preparation. However, it is always advisable to inform your doctor and the EVE Healthcare staff about any medications you are currently taking, especially blood thinners (anticoagulants), or if you have any known allergies. You can usually eat and drink normally before the procedure.

The FNAC Procedure: What to Expect

The FNAC procedure is typically quick and straightforward:

  1. You will be asked to lie down comfortably, and the area of the lump will be cleaned with an antiseptic solution.
  2. A local anaesthetic may be applied to numb the skin, though for many superficial lumps, this might not be necessary as the needle is very fine.
  3. The doctor will carefully insert a fine needle into the lump.
  4. Cells are then aspirated (drawn out) into a syringe. This process is usually repeated a few times to ensure an adequate sample is collected.
  5. The needle is withdrawn, and pressure is applied to the site to prevent bruising.
  6. The collected cells are immediately smeared onto glass slides and fixed for laboratory analysis.

The actual aspiration part of the procedure is very rapid, often lasting only a few seconds to a couple of minutes. The entire appointment, including preparation and post-procedure care, typically takes about 15-30 minutes.

Understanding Your FNAC Results

Once the pathologist examines the cells, a report will be generated categorising the findings. Results typically fall into categories such as:

  • Benign: Indicating a non-cancerous condition.
  • Malignant: Indicating the presence of cancer cells.
  • Atypical/Suspicious: Suggesting cells that are not clearly benign or malignant, requiring further investigation.
  • Inadequate/Non-diagnostic: Meaning not enough cells were collected for a definitive diagnosis, and the procedure may need to be repeated.

It's important to remember that FNAC provides a cellular diagnosis, which is a crucial step but often needs to be correlated with clinical findings and imaging results for a comprehensive diagnosis.
